FlourishCare Evidence-Based Program Initiative

The Trager Institute is proud to lead the FlourishCare Evidence-Based Program Initiative (FC-EBPI), a transformative initiative funded by a four-year, $600,000 grant from the Administration for Community Living (ACL). Through this initiative, the Trager Institute aims to enhance the quality of life for older adults in underserved areas of Kentucky. FC-EBPI offers evidence-based programs targeting some of the most pressing issues faced by vulnerable populations, including diabetes, dementia, and depression.

Our approach emphasizes person-centered care, helping individuals, caregivers, and families navigate complex health needs to achieve meaningful, sustained wellness.

Key Programs

The FC-EBPI incorporates three key programs designed to support diverse needs among older adults:

BRI Care Consultation (BRI-CC)

The BRI Care Consultation program focuses on supporting individuals with dementia and their caregivers. By addressing unmet needs, alleviating stress, and reducing depression, BRI-CC offers personalized guidance and support to help caregivers manage challenges and connect with resources in their community.

Diabetes Self-Management Program (DSMP)

DSMP provides participants with tools and strategies to manage diabetes effectively. Participants learn skills that promote lifestyle changes, enabling them to take control of their health and reduce complications associated with diabetes.

Program to Encourage Active Rewarding Lives for Seniors (PEARLS)

PEARLS is designed to help older adults manage depression and improve their emotional well-being. This program offers strategies to reduce symptoms of depression, encouraging older adults to lead active, fulfilling lives.

This program/publication is supported by the Administration for Community Living (ACL), U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $600,000 over 4 years with 100% funding by ACL/HHS.
