Our brain health clinic supports those with suspected cognitive decline and their loved ones. Our team, including dementia-trained care navigators and clinicians, works together to provide personalized care. We focus on creating a care plan that meets each person’s goals, strengths, and needs for a complete approach to managing dementia and related conditions.
How We Can Help
- Care Coordination: We help connect you with primary care providers, specialists, and any necessary care coordination services to ensure all aspects of your health are supported.
- Caregiver Support: We understand the challenges caregivers face. That’s why we offer training, access to community services, and a support line to reduce caregiver stress and promote their well-being.
Eligibility
If you or a loved one shows signs of cognitive decline, you can get help at our clinic. Even without a diagnosis, we provide assessments to confirm or rule out dementia.
For those already diagnosed with dementia, we offer the Guiding an Improved Dementia Experience (GUIDE) program, a new care approach from the Centers for Medicare and Medicaid Services (CMS). You can visit our GUIDE page to learn more.
