An Annual Wellness Visit (AWV) is a yearly health review for individuals, aimed at improving your overall wellness through prevention. This service is available from the comfort of your home via our telehealth program. 

For Medicare patients, the AWV is free and does not include a physical exam.

What to Expect

 During your Annual Wellness Visit, you and a member of your care team will:

  • Assess Health Risks to identify any potential health concerns.
  • Review Family History to understand your family's medical background.
  • Develop a Care Plan with an interdisciplinary team.
  • Screen for Key Areas of Health including physical, brain, and social health.
  • Conduct a Medical Review to ensure all your health needs are addressed.

After your visit, you will receive an Optimal Aging Care Plan that aligns with your health goals. Our interdisciplinary team will support you every step of the way.
