Better care for older adults, built alongside your team
The Care Hub is a statewide initiative designed to support skilled nursing facilities, long-term care and assisted living teams in delivering high-quality, age-friendly care.
Through a combination of training, hands-on support and ongoing collaboration, we work directly with your staff to address real challenges — whether that’s dementia care, workflow improvements, documentation or staff education.
Real Support. Practical Impact.
Our goal is simple: make high-quality care easier to deliver.
- Increased staff confidence in caring for older adults
- Improved care coordination and workflows
- Greater alignment with age-friendly practices
- Ongoing support — not just one-time training
