FlourishCare-Enhanced Internship Program

The FlourishCare-Enhanced (FC-E) Internship at the Trager Institute prepares students to deliver age- and dementia-friendly care across Kentucky through the Geriatrics Workforce Enhancement Program (GWEP). Interns gain hands-on experience providing age-friendly geriatric care across a range of settings, including primary care clinics, nursing homes and community-based organizations through FlourishCare Centers of Excellence (FC-CEs) that collectively serve communities statewide.
The program equips students for collaborative careers by integrating specialized geriatric knowledge and skills grounded in the 4Ms Age-Friendly Care framework—emphasizing patient-centered care, effective care transitions and interdisciplinary teamwork.
In addition, students engage with community networks led by Area Agencies on Aging (AAAs) and age-friendly steering committees, ensuring care delivery is coordinated, holistic and responsive to the diverse needs of older adults throughout Kentucky.

Meaningful, Hands-On Experience
Interns play an active role in making health care settings more age- and dementia-friendly by applying the 4Ms of Care—Mentation, Mobility, Medication, and What Matters Most.
Through partnerships with primary care practices, academic programs and community organizations, students contribute to improving care for older adults while building practical skills that translate directly to professional practice across Kentucky
Professional Training and Career Development
Interns receive interprofessional training focused on applying geriatric knowledge and skills in primary care settings. Students also gain access to career development guidance and job placement resources.
Financial Support
If grant funds permit, FlourishCare offers some limited stipend support to help offset the cost of participation.
