Who We Serve

- Mild cognitive impairment

- Caregiver concerns or care planning needs
A formal diagnosis is not required to be seen.

Family & Caregiver Support
We offer caregiver education, counseling, skills training and connections to respite and community resources to reduce stress and strengthen caregiver confidence.
Advance Care Planning
We help patients and families have early, meaningful conversations about values, preferences and future care decisions.
Legal & Resource Navigation
Guidance and referrals are available to assist families with legal planning and access to community-based services.
GUIDE Model & Care Management Services
For participants in the GUIDE (Guiding an Improved Dementia Experience) initiative, our program provides enhanced care management and caregiver support services aimed at improving quality of life and helping individuals remain safely at home whenever possible.
If you’re not eligible for GUIDE, we offer other care management options that your provider will discuss with you.
What is care management?
Care management supports patients as they manage their health care between medical appointments. Care management includes creating a customized care plan to address medical and non-medical needs, providing referrals, care coordination between provider offices, and access to caregiver support and educational materials.
Care management services may include:
- Personalized care planning
- Ongoing care coordination
- Medication review and safety monitoring
- Caregiver education and support
- Help navigating medical and community resources
- Access to support between appointments
