FlourishCare Centers of Excellence
Creating age-friendly health systems throughout Kentucky
The FlourishCare Centers of Excellence (FC-CE) bring together academic education and training systems, health care systems, community support networks and state organizations and associations to transform care delivery by integrating specialized geriatric knowledge and skills within primary care.
How they work
Area Agencies on Aging (AAAs) located in the FC-CE service areas are the community network leads, acting as central coordinators for community networks in the FC-CE regions to ensure all systems collaborate effectively to provide comprehensive patient-centered and team-based care. The AAAs are supported by a steering committee dedicated to age- and dementia-friendly initiatives, including representatives from all partner entities and older adult advocates.
There are currently three Centers of Excellence serving the counties located in the Kentuckiana Regional Planning and Development Agency (KIPDA), Barren River Area Development District (BRADD) and Cumberland Valley Area Development District (CVADD) regions.
Steering committees
The steering committees work in collaboration with their local AAA and community partners to identify and eliminate barriers to transitions of care for older adults.
Participants contribute by
- Offering input on strategies to improve coordinate care when older adults move between hospitals, rehab centers and home
- Collaborating with state and local government, health care systems, academic institutions and community organizations

An age-friendly system demonstrates a commitment to the evidence-based 4Ms framework for older adult care.
The 4Ms
What Matters Most
Know and align care with each older adult's specific health outcome goals and care preferences including, but not limited to, end-of-life care, and across settings of care.
Medication
If medication is necessary, use Age-Friendly medication that does not interfere with What Matters Most to the older adult, Mobility or Mentation across settings of care.
Mentation
Prevent, identify, treat and manage dementia, depression and delirium across settings of care.
Mobility
Ensure that older adults move safely every day in order to maintain function.
