Diabetes Self-Management Program

Diabetes Self-Management Program (DSMP) is a science-based six-week program to self-manage a diabetes or pre-diabetes diagnosis.

Who it's for

DSMP is open to anyone with a diabetes or pre-diabetes diagnosis. Preference given to those aged 60 and older.

  • Sessions: 6 sessions
  • Format: Sessions held over Zoom. Computer with audio and video are required.
